Housecall Doctors PC is part of the American Academy of Home Care Medicine since 2010. We have focused on home-based primary care practice, making medical house calls to older adults and homebound patients for more than 23 years. Housecall Doctors utilizes the latest technologies to the advantage of our patients. We currently provide comprehensive blood testing, remote patient monitoring, culture screening, as well as mobile X-ray, EKG, and ultrasound diagnostics at home. Other services include immunizations such as flu and pneumonia vaccinations. We accept most Medicare, Medicare Advantage, and Medicaid Plans
Home Medical Visits
Our patients who are looking for a primary care provider are seen regularly with the aim to prevent hospitalizations and/or hospital readmissions. As a primary care provider, Housecall Doctors will make sure you are cared for from A to Z, including sick visit requests or Coumadin monitoring.
Hospital Discharge
To reduce the rates of readmission, Our medical providers will make home visits to patients post-discharge to monitor patients. They discuss durable medical equipment, medication needs and education, nursing care, primary care provider appointments, physical therapy, and any additional post-discharge services needed.
Nursing Home Placement
Assisting families with placement in adult care homes, nursing homes, assisted living, and other residential health care settings or institutions. Aging and disabled adults receive help to complete medical evaluations, monitoring, and counseling to help them adjust to change.
Wound Management
Our certified wound care providers with experience in direct patient care work closely with home health partners to care for diabetic foot ulcers, pressure injuries, venous leg ulcers, cellulitis, and other non-healing or infected wounds
Palliative Care
Our palliative care provides symptom relief, comfort, and support to people living with serious illnesses like heart disease, cancer, or chronic respiratory disease. Its benefits extend to caretakers, too. Your care team works to deliver the best care outcomes available for your condition.
Hospice Care
Our hospice care service is dedicated to providing you with dignified care that promotes quality of life, comfort, and convenience. We respect your personal healthcare and treatment decisions and strive to support you and your family with your choice of hospice care.
